# Forms

A **form** lets people outside your organization — donors, volunteers, applicants, board members — send you answers and files. Anyone with the link can respond; they don't need a WIZE account. Every response becomes a row in a spreadsheet, and every uploaded or recorded file lands in a folder in your Documents.

Forms live on [designs](/help/documents/#designs): a flyer, invitation or one-pager gets a button that opens the form.

## Making a form

**Ask WIZE.** In chat, say what you want — "add a volunteer sign-up form to this flyer with name, email and preferred shift" or "make an RSVP form for the gala invitation". WIZE creates a real, working form on the design and places its button in the design for you. To change it later, just ask: "make the phone question optional", "add a question about dietary needs", "close the form".

**Or use the Forms button.** Open the design and click **Forms** in the design's header. Click **New form**, give it a name (it is also the text on the button in your design), add your questions, and click **Save form**. Then click **Add to design** to put the form's button at the end of the design — ask WIZE to move or restyle it.

A design on its own can't collect anything: if you see text boxes drawn into a design, they are only a picture of a form. A working form is always a button or link that opens the form.

## Question types

**Short answer**, **Paragraph**, **Email**, **Phone**, **Number**, **Date**, **Multiple choice** (pick one), **Checkboxes** (pick any), **Yes / no checkbox**, **File upload**, **Audio recording**, and **Video recording**. Any question can be marked required.

Respondents can attach up to 5 files per response, each up to 100 MB. Audio can be recorded right in the form; video is recorded on the form's own page (the form offers a link to it).

## Sharing a form

- **In the design.** Clicking the form's button — in WIZE or on the design's public share link — opens the form over the design.
- **As a link.** In **Forms**, **Copy link** gives you the form's own web address to paste into an email, a social post or a QR code.
- **In a PDF or print.** The button is an ordinary link, so in a downloaded PDF it opens the form's page.

## Where responses go

- **Answers** go to a spreadsheet named "*form name* — responses", one row per response, one column per question. **Open responses** in **Forms** opens it. You can sort, edit and analyze it like any spreadsheet — and ask WIZE about it.
- **Files** go to a folder named "Form responses — *form name*". The spreadsheet row links to each file.
- **Notifications.** You get an in-app notification for each new response.

If you delete the spreadsheet or the folder, WIZE creates a fresh one with the next response.

## Opening and closing

In **Forms**, the **Accepting responses** switch opens or closes a form — or ask WIZE to close it. A closed form tells respondents it is no longer accepting responses; your existing responses stay where they are.

## Privacy

A form on a **private** design gets a private spreadsheet and folder that only you can see. Otherwise they follow your workspace's default for new documents. If the person who made a form leaves your organization, the form keeps running and its responses are shared with your workspace's admins.
